What to Prepare Before Your First Visit
Welcome to Smileworks Dental! We are so excited to meet you. To help us make the most of your first appointment, we ask that new patients complete the forms below before arriving at our Calgary dental clinic.
If you have any questions about the forms or about your upcoming visit, please do not hesitate to contact us. You are also welcome to arrive a few minutes early on the day of your appointment to complete your forms in person.
